RED CARD & MMO REPORT FORM (from Sept 2011)

Description This form is to be completed by the umpire following the issuing of a Red Card or to report a Matchday Misconduct Offence (MMO).
The other umpire may comment if they wish – see section 15 on the form.
The completed form should be signed and dated, and sent as soon as soon as possible (and in any event within 72 hours of the offence) by email or First Class post to the relevant Disciplinary Administrator.
